Care at the End of Life Collaborative

Approaching the end of life is a significant and sensitive time for people with a serious illness and their loved ones.

A new website, WestMoretonCareConnect.com.au, is launching to help people connect with local services, support networks and organisations that can assist people reaching the end of their life.

This is the first of its kind in West Moreton and CiMaS is a proud member of the West Moreton Care at the End of Life Collaborative, which is behind the website.

It includes information on everything from health and wellbeing services, enduring powers of attorney and advanced health directives, to assistance groups and services that will mow your lawn or provide taxi vouchers or meals.
